While we prefer
to "custom-draft" Power of Attorney for Health Care documents,
when we do estate planning documents for our clients, we recognize
some people may prefer to do their own documents. A variety of "do-it-yourself"
documents can be used for these purposes. If you are interested
in such documents and in obtaining further information on the subjects,
the following are at least a few places you may want to look:
www.cmanet.org
California Medical Association (CMA), CMA Publications, PO Box 7690,
San Francisco CA 94120-7690
www.finalchoices.calhealth.org
Finding Your Way, Sacramento Healthcare Decisions, 4747 Engle Road,
Carmichael CA 95608 (916) 484-2485
www.seniorlegalhotline.org
Senior Legal Hotline, 515 12th Street, Sacramento CA 95814 (916)
551-2145
www.agingwithdignity.org
Aging With Dignity, PO Box 1661 Tallahassee FL 32302-1661 (850)
681-2010 (publishes and distributes the "Five Wishes"
document, which is not approved for use in California)
www.partnershipforcaring.org
Partnership for Caring, Inc., 1035 30th Street NW, Washington
DC 20007-3823 (800) 989-9455
Since I believe
each adult should have a Power of Attorney for Health Care document,
anyone who requests one will be provided a California Power of Attorney
for Health Care document, as provided by California Probate Code
Section 4701. This document is a "statutory" health care
document and can be used by you to put an advance health care directive
in place. If you follow the instructions in the document to complete
and sign it, you will at least have a basic document in place.
You can download
the form or call our office at (916)965-8941.
